Fosfomycin, IUPAC name (–)-2-amino-1-phosphonoyl-methyloxirane, is a phosphonic acid derivative with molecular formula C3H7NO4P and molecular weight 152.11 g/mol. Its structure features a β-phosphonolactam ring, conferring stability against hydrolysis. Therapeutically, it treats urinary tract infections caused by Gram-positive and Gram-negative bacteria. Physicochemically, it is a white crystalline solid, water-soluble (pKa ~1.8), and resistant to β-lactamases. Mechanistically, fosfomycin inhibits bacterial cell wall synthesis by competitively inhibiting UDP-N-acetylglucosamine enolpyruvyl transferase (MurA), blocking peptidoglycan precursor formation.
