Diltiazem, a calcium channel blocker, is chemically known as 1,2,3,4-tetrahydro-5-[[2-(dimethylamino)ethyl]methylamino]-2,4-dimethyl-6-(p-methoxyphenyl)-1-(2-propenyl)pyridine-3,5-dione (C22H26N2O3; molecular weight 366.46 g/mol). It belongs to the benzothiazepine class, featuring a pyridine ring fused to a thiazepine ring with methoxyphenyl and dimethylaminoethyl substituents. Therapeutically, it treats hypertension, angina, and arrhythmias. It is a white to off-white solid, slightly water-soluble, with a melting point of 130–135°C. Diltiazem inhibits L-type voltage-gated calcium channels, reducing intracellular calcium influx in cardiac and vascular smooth muscle, thereby decreasing myocardial contractility and vascular tone.
